The Washington state Department of Health (DOH) has taken disciplinary action or suspended credentials from four area health care providers, including a former Green Hill School employee, according to recent releases from the agency.
Thurston County
In August, the DOH indefinitely suspended Leandra Dishan Calhoun’s agency-affiliated counselor and substance use disorder professional trainee credentials.
Calhoun is a former employee at Green Hill School. In June 2022, Calhoun was arrested regarding her alleged involvement in a drive-by shooting that occurred in June 2022.
According to the DOH, Calhoun did not cooperate with the agency’s investigation into the incident or the related allegations of unprofessional conduct — and failed to respond to multiple follow-up requests for information.
Also in August, the DOH indefinitely suspended April Dawn Irene Hale’s certified nursing assistant credential. According to the DOH, between 2019 and 2023, Hale misused a patient’s resources for personal benefit and failed to respond to the department’s request for information.
In July, the DOH indefinitely suspended Jonathan Mark Hoyer’s agency-affiliated counselor and independent clinical social worker associate credentials. According to the DOH, Hoyer failed to comply with a 2025 order by not participating in a required monitoring program.
Lewis County
In August, the DOH charged certified medical assistant-phlebotomist and certified nursing assistant Sara Diana Vasquez with unprofessional conduct. In 2021, Vasquez was convicted of two counts of driving under the influence. According to the DOH, Vasquez acknowledged the convictions in a 2023 credential application but did not notify the agency within the required 30 days.
